Community Health Worker - Infection Disease- Full Time (40 Hours)

Job Description
- This role serves as a liaison, link or intermediary between health, social services and the community to facilitate access to services, and improve the quality of service delivery.
- This role also serves as an advocate for patients, a resource, and a vital part of the interprofessional healthcare team. Educates patients, engages and assists them in managing their healthcare needs.
- Provides non-clinical paraprofessional duties and is thoroughly familiar with the patients community and the resources available.
- Collaborates with healthcare staff, community based healthcare, social service and other providers.
EDUCATION/EXPERIENCE REQUIRED:
- High school diploma or G.E.D. equivalent. Previous experience as a community health worker, preferred.
- Previous experience conducting chronic disease education classes, preferred.
- Previous experience conducting home and/or clinic visits with patients and their primary care providers, preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Certification as a Community Health Worker or a willingness to become certified.
