Provide case management services for the clients of The Salvation Army Harbor Light
Summary:
Facilitate didactic groups and case management services
Primary Duties and Responsibilities:
Conducts didactics/educational lectures
Maintain case records in accordance with Harbor Light standards
Serve as Case Manager to treatment clients
Assists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)
Participates in the review of all clinical issues, activities and program policies and procedures
Attends all clinical staff meetings, and team conferences
Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills
Qualifications:
Ability to determine needs and to work with multi-problem individuals
Ability to develop rapport, trust and confidence in client relationships
Must possess good communication and teamwork skills
Strong organizational skills
TB Test and police clearance required
Authorization to work within The United States
Education/Experience:
A bachelor's degree in social work or an equivalent comparable human services field is required
Experience in case management preferred
Experience with mental health and/or substance abuse populations preferred
Computer Skills:
Excellent computer skills required
Experience using electronic medical records systems preferred
Reasoning Ability:
Understand and work within The Salvation Army and Harbor Light Mission Statements
Possess an excellent working knowledge of The Harbor Light Policies and Procedures
Certificates and Licenses:
Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
Valid Michigan Chauffeur License and approved by The Salvation Army MVR
